Although initially applied to the prevention of recurrence in unipolar depression, training in mindfulness is now seen as helping build and consolidate generic emotion regulation skills across a variety of emotional disorders. The key themes of this approach include experiential learning and the development of an open and acceptant mode of response, in which one intentionally faces behavioral difficulties and affective discomfort. With respect to disorders of anxiety, mindfulness may be seen as ‘exposure light’, when in fact it is positioned far earlier on the emotion regulation continuum, affecting attention deployment and response selection. This lecture will present the scientific rationale for training anxious patients in mindfulness meditation as well as discuss the strengths and weaknesses of empirical support for this approach. Finally, clinical choice points regarding its use for treating acute symptoms versus sustaining treatment benefits and its integration with traditional behavioral and cognitive formats will be offered.
